| The CIME-PC Program is an innovative approach that will bring together the knowledge, resources, and talent of interdisciplinary team members, as well as three UT Medical Schools (UTHSCSA, UTMB - Galveston and UT Health Science Center - Houston) to provide long-lasting value added impact on physician education in the competencies needed to care for chronically ill and terminally ill patients and their families.
MS 2 will work more with total pain concepts and interview a real patient as a ateacher. My teaching with this group is once a year
MS 3 work with delivering bad news and communication skills and a total pain case. Four times a year

| A two hour learning session was offered on site at three different times to RNs, LVNs, Physical and Occupational Therapists and their assistants, nutrition workers, case managers, SW and CNAs at a small community hospital. Attendance was mandatory. The educational sessions were designed to be case based and participatory to provide for maximum interaction between workers of different disciplines.

| Residents spend one half day each month in LTC for 3 years. This rotation covers administrative and clinical aspects of nursing home care. Rounds are used as educational sessions using a case based approach. I coach residents on taking LTC calls and on communicating with families. Topics include dementia, pressure sores, rehabilitation, falls, incontinence, infection control, malnutrition, cancer, and transitions of care. |

| I coordinate weekly Geriatric Grand Rounds for the interdisciplinary team at the Nix Hospital. Participants include nursing, infection control, nutrition, trainees, and primary care physicians. Grand rounds support our mission in teaching, service, and research in geriatric care. Rounds rotate between guest speakers, educational sessions to improve curriculum, morbidity and mortality grand rounds, journal club and billing expertise. |

| Third year Family Practice residents and 6 Fort Hood Family Practice residents came for an entire month and cared for skilled nursing patients. This rotation is mandatory for the residents. This rotation covers administrative and clinical aspects of nursing home care. Topics are reviewed monthly with the residents, rounds are educational using the patients as a case based approach. I also teach residents on how to handle nursing calls and how to communicate with families and transition of care(hospital to nursing home and vice versa). Topics are longitudinal and include dementia, pressure sores, rehabilitation, falls, incontinence, infection control,malnutrition, cancer etc. I spend 20% (2 half day sessions)a week. |

| All third year Family Practice Residents rotate an inpatient care for OLDER ADULTS only. The resident spends one month per year. The curriculum includes dementia in an acute setting, delirium, managing medical errors, polypharmacy, palliative care and end of life discussion with families, family meetings, radiology rounds, how to work with consultants and interdisciplinary approach in acute settings, discharge planning etc.
I rotate 2 months a year in the inpatient and always with a resident on board and a fellow once a year |

| Nix Senior Health Center clinical rotation is a site for training residents from our family and Community Medicine department, the ob-gyn residents, Family Practice Darnell program from Fort Hood and also for elective residents from Santa Rosa Program and for students and residents from other sites in the country offer as an elective. I precept this clinic 30%, discussing geriatric outpatient topics relevant to the outpatient clinic. Topics include dementia, geriatric assessment, caregiver burden, polypharmacy, osteoporosis, incontinence, etc. |

The GACA supports my career development as a clinician educator by giving me the resources needed to focus on a challenging educational project. I developed educational skills by creating and implementing a multidisciplinary curriculum in acute care of the elderly using the ACE without walls model. This model cares for older adults at the hospital level and focus in preserving function and fast recovery using an interdisciplinary approach; (2) develop and apply my leadership skills in the process of implementing ACE without walls in a small private hospital, and (3) acquire the skills to conduct educational research in order to document, evaluate, and disseminate the results of my endeavors through publications and national and international presentations. |

The Health Science Center Preparatory Academy (HSC-Prep) was created to improve performance on the SAT Reasoning and Subject Tests among local high school students. The goal is to increase the number of qualified applicants to the health professions from economically disadvantaged ethnic minority communities. Dr. Oakes will contribute 5% time for a six month period January-June of 2008 and 2009 to develop and revise curriculum. Dr. Oakes will develop five geriatric topics each into one hour sessions with the goal of providing instruction for the students on core subject skills including reading comprehension, writing, mathematics, biology and Spanish language.Every year Dr. Oakes and fellows will deliver the program to close to 100 undeserved students.
So far 2 fellows have contribute in this activity. |
