Denise R. Dahm, MD, MPH is an Associate Professor of General Internal Medicine and Geriatric Medicine at the University of Texas Health Science Center at San Antonio. Dr. Dahm joined the faculty of UTHSCSA in July 2009 as a clinician educator in the Division of Geriatrics, Gerontology and Palliative Medicine. She has been involved in geriatrics education for medical students, residents and fellows for the past eight years. She served as Associate Program Director of the Geriatric Medicine Fellowship from 2012-2015. In this role she chaired the Clinical Competency Committee for the fellowship, developed and implemented new curricula such as the transitions of care curriculum, and served as a career and clinical research mentor for geriatrics fellows. She also built a thriving outpatient practice at UT Medicine`s MARC Geriatrics Clinic in addition to creating a transitional care clinic to provide rapid discharge follow-up for patients recently discharged from hospital to home. In October of 2014, she transitioned to a position as a faculty attending in the internal medicine resident`s continuity clinic at the Robert B. Green Downtown Campus, where she has continued to provide training in geriatrics, establishing a Geriatric Assessment Clinic in July 2016. She has presented at local, regional and national meetings on transitions of care, delirium, and comprehensive geriatric assessment.
