Ruby Mathew is an Acute Care Nurse practitioner working with the UTHSCSA Family & Community Medicine. She is a member of of geriatric and palliative care team. She functions as a clinician, educator, and coordinator. Primarily she works at the Acute Care for the Elders( ACE) unit at the Christus Santa Rosa Hospital and takes the lead in Interdisciplinary Team Rounds(IDT) and takes part in ACE workshop which are aimed to teach the nurses about geriatric friendly care during the hospitalization. In addition, she is running a Transition of Care (TOC) clinic in the UT Senior Health Clinic; with the focus of reducing hospital readmissions and medication errors. During her three years of service with UTHSCSA, she participated in the clinical safety and effectiveness course to focus on QI projects to improve the number of TOC patient visit to clinic. Also, she was the recipient of clinical excellence award in the Year of 2015. In the year 2016, her ongoing focus will be on TOC clinic more geared toward palliative care patients.
